The Child Health Service send out reminders when immunisations are due. Please call reception to arrange an appointment with a Practice Nurse and also mention the vaccination you are booking for.
All newborn babies should have a six to eight week check with the Doctor.
Immunisation Schedule
2 Months
Diphtheria, tetanus, whooping cough, polio, Hib and hepatitis B, Rotavirus, Meningitis B.
3 Months
Diphtheria, tetanus, whooping cough, polio, Hib, hepatitis B, Rotavirus and pneumacoccal.
4 Months
Diphtheria, tetanus, whooping cough, polio, Hib hepatitis B and Meningitis B.
12 Months
Hib and Meningitis C booster, Measles Mumps and Rubella, and a pneumococcal booster, Meningitis B.
3 Years 4 Months
Diphtheria, tetanus, whooping cough, polio Booster and Measles, Mumps and Rubella booster.
Timing of Appointments
You will receive a reminder letter from the Child Health Service when your child’s immunisations are due. Please contact the surgery to make an appointment.
- Please arrive on the correct day and at the correct time slot for your appointment
- Please do not arrive early or on a different day
- Try to bring only the relevant child, when possible
- Bring your appointment letter and Child Health Record to your appointment
Consent
If anyone other than you, as the parent, is bringing your child, they must have signed consent for the immunisation from you as the parent or guardian.
Cancellations
If you are unable to attend your appointment please ring the surgery to cancel and rebook.
Other Information
If you have any concerns or questions regarding your childs immunisations, please
discuss these with your Health Visitor prior to your appointment day.
In the event of an emergency or other extenuating circumstances, the clinic may be cancelled without notice. If this occurs we will offer alternative appointments.
For everybody’s comfort and safety, we would be grateful if you would leave the surgery following your childs immunisation as space is extremely limited.
