New Patients

If you would like to apply to register at Highlands Surgery, please visit

All forms should be completed and returned to the surgery by hand to then compete a purple final form at the surgery. The sample container(s) should be retained for when you attend for your New Patient Appointment.

Please contact the surgery after 5 working days (between 2.00pm and 5.00pm wherever possible) to confirm the registration has been processed and to arrange your New Patient Appointment with one of the Practice Nurses.

Change of Name and Address

The Health Authority asks that we have official documentation to change names, revert back to a previous name and/or if there is any discrepancy in dates of birth.

If you change your name or move house, please notify us as soon as you can.

To help keep your contact details up to date please let us know if you change your landline or mobile telephone number.

Text Message Reminder Service

If a mobile telephone number is registered a text message is sent as a reminder for any appointment booked. Please let the Surgery know if you do not wish to receive text messages.


All specimens should be at the surgery by 12.00 noon (Monday to Friday), for the hospital to collect them to take to the laboratory.

After 12.00 noon, all specimens must be taken directly to the hospital.

Time to Learn

The surgery is closed one afternoon/evening each month (except January/August) for the development of professional skills for GPs, Nurses and other Health Care staff. This is usually held on the first Tuesday of every month unless there is a bank holiday in which case it will be the second Tuesday of the month.

Dates will be displayed in reception.


We have limited number of parking bays which may be used as and when available. Please be aware of pedestrians accessing the building. Please park considerably around the residential area of the surgery.

Disabled Access

The surgery has access for a wheelchair, a lift to the first floor and a disabled persons toilet is available adjacent to the Nurse Room on the ground floor.
