In a world of technology and digital transformation we are aware that a group of more frail possibly older patients may struggle.
We have two nurses, Amy and Lisa, that do Admission Avoidance work with those identified as needing extra support.
They have links to the Primary Care Network and allied care workers like district nurses and palliative care.
We proactively identify and contact those in need of additional support and monitoring, and often receive information from ambulance services or hospital discharge letters that need additional care.
